The following forms are available in PDF format. You will need a PDF reader like Foxit Reader (recommended). The forms are fillable, just fill out the form, then you can print it. These forms must accompany the standard enrollment form before the child is enrolled.

- CACFP Enrollment Application
- Enrollment application for centers with no meal charge.
- Consent for Emergency Medical Treatment
- Authorizes Wonder Years Child Care Center to obtain emergency medical or dental care.
- Health Policy
- Must be signed by a Parent or Guardian acknowledging reading of the Health Policy.
- Indentification and Emergency Information
- Lists persons and physicians to be called in case of an emergency.
- Medical Statement
- For participants with allergies and/or chronic diseases.
- Parental Rights
- Notification of Parent's Rights to be completed by a parent or guardian
- Personal Rights
- Full disclosure of Personal Rights to be completed by a parent or guardian
- Physician's Report
- Pre-admission health evaluation to be completed by the child's physician.
